Home care tailored to your needs

Where your needs are deemed complex and you require further clinical care, we can provide Specialist Registered Nurses to support your personalised package of care in the comfort of your own home.

A package delivered by Nurses will allow for clinical decisions to be made based on the ongoing and changing needs of the care that you receive.

Creating your bespoke package of care

When developing a bespoke package of care, we will work with you and your family where appropriate along with multidisciplinary teams to create a package of support that meets your clinical, social care and personal needs. We often work closely with hospital units and acute settings at the start of your journey to facilitate your transition home.

We consider the practical and clinical elements of your care alongside the emotional and social support aspects associated with daily life. We have vast experience of delivering complex care in the home and this has equipped us to support our clients and their families during what can be an incredibly challenging time.

Our Registered Nurses come highly trained and experienced in their field of expertise and are registered with the Nursing and Midwifery council (NMC). They can use their clinical decision-making skills to ensure that your care plan adapts with your changing needs and requirements.

How we support you

The care we can provide for you at home is with our Registered Nurses. The type of care we provide for you at home includes:

  • Provision of trained Nursing support to deliver the clinical complexities to your package of care.
  • Development of a personalised care plan that our Nurses can work with and update in relation to your changing needs or requirements.
  • Supporting with the initial discharge from a hospital or rehabilitation unit.
  • Supporting multiple and complex clinical needs.
  • Development of confidence towards greater independence and involvement within the local community.
  • Assisting you with any health needs if required.
  • Providing respite and additional support to your family.

HPN and IV Therapies

There is a steady increase in the number of children and adults receiving home enteral tube feeding and home parenteral nutrition.  Home Parenteral Nutrition (HPN) provides food and fluids through Intravenous (IV).

HPN is needed for service users with acute or chronic intestinal failure in whom nutritional and/or water and electrolyte balance cannot be corrected by oral or enteral feeding and in whom HPN is feasible at home (Messing B et al in Intestinal Failure 2001).

We can deliver Home Parenteral Nutrition and Intravenous (IV) Therapies as part of the care packages that are delivered by our Registered Nurses.

Getting you home

Where a quick discharge home from hospital is required, we can provide Registered Nurses to provide hands on care to support you with this. Once you are back in the comfort of your home, we can continue to train your Healthcare Assistants, where required, in your personal, clinical, and social care needs.

Our Registered Nurses will work as part of the care team to ensure you receive the highest quality of personalised care.

This is just a quick note to say how impressed and thankful I was to the girls who were here on Christmas day.
