Job description

Apollo Home Healthcare currently has a position available for a Care Coordinator to join our office based in Norwich.

We provide complex care in the home of individuals who have complex and life-limiting conditions. This includes adults, children and the elderly and conditions range from Cerebral Palsy, brain and spinal injuries and complex genetic conditions.

As a Co-ordinator your key responsibilities will be;

  • Managing a caseload of complex care packages dealing with all operational tasks associated with them.
  • Building relationships with clients and care staff to ensure the seamless delivery of all packages of care
  • Ensuring all packages have strong care teams along with necessary contingency plans managing rotas, supervisions, spot checks and appraisals
  • Liaising regularly with clients and agency workers reporting any complaints/issues of concern to the Registered Manager
  • Producing weekly/monthly reports in relation to service delivery
  • Attending visits with service users and their families to review levels of service and deal with any requests
  • Carrying out care plan reviews for caseload alongside Office Nurse.
  • Ensuring all practices and processes are in accordance of current legislation and within corporate policies and procedures

Our Norwich office is an extremely busy and fast paced environment, so we need someone who can take a hands-on approach with the ability to problem solve and work off their own initiative.

You must have previous experience working within the care sector, ideally within a complex setting. Strong communication skills are essential as you will be liaising with care team, clients and in Clinical Commissioning Groups.

Travel will be required so you must also hold a full current driving licence valid in the UK and have access to a car.

In return for your hard work and dedication you will receive a salary of £18,000 - £25,000 per annum and;
