How we deliver our care
We follow a six-step process to create a bespoke care plan that is tailored to meet your individual needs.
Step 1: Working together
We will hold an initial telephone conversation with you (your family, advocate or other health care professional involved in your care), to help us understand your specific care needs. We will provide you with further information about our services and discuss how our care service can be tailored to meet your individual clinical, personal and social care needs. A quotation can be provided to you on request.
Step 2: Assessing your personal needs
We offer a free home visit with a Registered Nurse so that we can discuss your care needs in more detail, and to fully understand your home set up. We will also undertake a general risk assessment of your home so that we can provide you with a service proposal that is designed to meet your specific care needs. We will then provide you with a final quotation for your personal care plan with a detailed breakdown of our service proposal and delivery.
Step 3: A care plan tailored just for you
Once you are ready to proceed, we will finalise your personal care plan which will include the care you will receive, how the care will be delivered and most importantly your personal preferences about how you would like your care to be provided. Once you (your family, advocate or other health care professional involved in your care) are completely happy with your care plan we will bring together your personal care team.
Step 4: Your care team
Our specialist inhouse recruitment team will use their expertise to build you a bespoke care team, matching our experienced and skilled carers to your individual needs. We can recruit new care staff specifically for your package of care as well as looking at our existing staff that are already trained in your individual care needs. We will introduce your carers to your specific package of care and start the delivery of your service at a date and time agreed by you.
Step 5: Training
Our Healthcare Assistants and Support Workers will go through a meet and greet, shadowing and specialist training to ensure that they have the skills required to support the care that is required. Our Registered Nurses spend time in your home to ensure that staff are fully trained and competent to carry out each clinical task. We provide full Registered Nurse oversight to all the packages of care that we deliver.
Step 6: Ongoing support
Rest assured our service doesn’t end here. Once your care package commences, we will be in contact with you regularly for any feedback about the care that is being delivered. We will also undertake quality checks to ensure you continue to receive the highest quality service from our team.
