How we work with case managers

When we create our care, support, and rehabilitation packages we focus on the individual clinical, personal and social care needs of your client along with their personal preferences, which enables us to find the right healthcare team for them. We also work with your client’s relatives to introduce care in a way which supports family life.

Working with a team of locally based care managers and clinical nurse specialists with expertise in condition-based care, we will visit prospective clients to build an individual detailed care plan.

Where a client is residing in hospital or a care setting, we work alongside healthcare professionals and multidisciplinary teams (MDTs) to understand the complex healthcare needs and clinical requirements of the client to ensure continuity of care when transitioning back home.

Following our assessment, we will write up our recommendations for the care package, establish final quotations and discuss essential budget projections. During this stage we can also advise on and organise any specialist equipment or room adaptations and select suitably trained care professionals who will be matched to your client by both skills and personality.

We will provide you, as the Case Manager, with accurate and timely information in a format that can be easily included in reports and recommendations that you may be making on behalf of your client.

Our commercial team will provide you with full case management support throughout the whole process, quoting, advising on, organising, recruiting to, and implementing seamless packages of complex home healthcare for adults, young people, children and babies across the UK.

Personal Injury and Medical Negligence Clients

We understand that clients who have sustained personal injuries can have differing complex healthcare, rehabilitation, and social care needs right from the beginning of their journey, or through the litigation process than our more traditional continuing healthcare clients.

Therefore, we have developed our services specifically for those affected by personal injuries and those who are managed by case managers, personally injury representatives and solicitors.

Our complex home healthcare services are available right across England and coordinated by our locally based care managers and clinical nurse specialists, who all have expertise in condition-based care from our five local offices.

Pre or post settlement cases

We understand that a care case manager may need initial care package costs to support the litigation process and we regularly receive and process enquiries that require a speedy response.

Our team will work quickly to determine and deliver comprehensive and fair quotations, which can then be presented to solicitors. We’ll conduct assessments by telephone and/or face-to-face where possible within the timeframes given.

For case managers working with post-settlement cases, we can establish your clients’ needs and implement bespoke, personalised packages of care in accordance with your timescales and requirements.

Once these packages have been implemented, our locally based managers, clinical nurse specialists and fully trained care professionals will work alongside case managers to ensure that the best outcomes are achieved for your client.

Where there is a need to discharge a patient within a restricted time frame, we have experience of implementing innovative measures to ensure that a faster discharge does not compromise on safety or quality of care.

We take the stress away by managing the whole process from quotation and referral, right through to facilitating the discharge home, and then the continued ongoing day to day management of the package of care.

Funding

Many of our clients are in receipt of dual funded packages, either privately funded, case management settlements or have personal health budgets.

Clinical oversight in our nurse-led service

Our nurse specialists provide clinical oversight for the entirety of the package of care, from the planning and quotation stages, carrying out complex care case assessments, right through the lifecycle of the package of care with regular care reviews and assessments.

They hold a range of specialisms and support our adults, young people, children and babies with a wide range of complex life-limiting and long-term conditions. They play a pivotal role in providing clinical oversight and governance to the packages of care we deliver for our clients, ensuring they can live as independently as possible at home and enjoy the things they love to do.

They ensure our care professionals are expertly trained, including any specialist training required, and that they are both competent in their clinical skills and confident in delivering only the highest-quality complex care in the home. They undertake regular competency assessments to ensure continued high standards of care delivery and provides clinical monitoring, supervision and support throughout.

With this leadership and oversight, our carers can perform many delegated healthcare tasks and clinical interventions at home, negating reliance on community district nursing teams.

Our care professionals are trained to administer clinical tasks, including:

Respiratory and Airway Support

  • Oxygen delivery and suctioning.
  • Cough assist working in partnership with physiotherapists.
  • Tracheostomy management.
  • Non-invasive Positive Pressure Ventilation (NIPPV) via masks – CPAP/BiPAP.
  • Invasive ventilation via machines – Nippy.

Feeding and Supplementary Support

  • NGT/PEG/gastrostomy management.
  • Continuous/intermittent pump feeding.

Elimination and Toileting

  • Catheter care.
  • Stoma care.

Fully vetted and expertly trained
Every one of our care professionals is fully DBS-checked along with a range of compliance checks before they start working with us. They receive full clinical training and any specialist training, alongside shadowing before being signed off as competent by one of our clinical nurse specialists, which equips them to provide the wide range of specialist complex care services and support services that we offer.
